> MySpermBank Donor Program recruits and screens sperm donors for intended parents seeking donor conception. Part of Inception Fertility and The Prelude Network®, we accept less than 1% of donor applicants through rigorous medical, genetic, psychological, and background screening. Donors receive compensation up to $1,500/month, contribute to helping families achieve their dreams, and participate in programs including Early Disclosure, Open ID, and Known Donor options. The donation process typically takes 6-12 months from application to approval, with ongoing donations 1-2 times per week.

## Donor Requirements & Screening

MySpermBank has strict eligibility criteria:

- Age 19-39 years old
- Healthy lifestyle and medical history
- No family history of genetic conditions
- Able to commit to 1-2 donations per week for 6-12 months
- Pass comprehensive medical examination and infectious disease testing
- Complete expanded genetic carrier screening (500+ conditions)
- Undergo psychological evaluation and counseling
- Pass criminal background check and identity verification
- Excellent sperm quality with good post-thaw motility

Screening process includes:
- Initial application and questionnaire
- Semen analysis for count, motility, and morphology
- Blood and urine testing (HIV, hepatitis B/C, syphilis, CMV, etc.)
- Genetic screening and genetic counselor consultation
- Karyotype chromosome analysis
- Psychological assessment with validated tools (MMPI or PAI)
- Three-generation family medical history review
- Federal criminal background check
- Education and identity verification

## Donor Identity Options

- **Early Disclosure Donor**: Identity shared with family after live birth is reported
- **Open ID Donor**: Identity shared with donor-conceived child at age 18 upon request
- **Known Donor**: Meet intended parents before donation through facilitated introduction (limited availability)
